To the Medical Officer of Health,
I hereby give you notice of the birth of a live (or stillborn)

N.B.—The above form must be filled in and delivered or posted to the Medical Officer
of Health, at the address on the other side within 36 hours after the birth
of any child in the Borough.
The persons required to notify by the Notification of Births Act are the father
of the child, if he resides at the time in the house where the birth took place, and
any person in attendance upon the mother at the time of, or within 6 hours after
the birth.
This notification applies to any child which has issued forth from its mother
after the expiration of the 28th week of pregnancy, whether dead or alive.
In addition to sending this notification the persons responsible
must register the birth with the Registrar in the usual way.
