Note.—New cases first coming to knowledge otherwise than by formal notification may in some instances
afterwards be formally notified on Form A or Form B. Such cases should be included in the appropriate
columns of the Summary of Notifications overleaf and not in the Supplemental Return, unless the formal
notification in any of such cases was improperly made, e.g. after death of the person, in which event the
case should appear in the Supplemental Return.
No case should be included both in the Summary of Notifications and the Supplemental Return.
