(a) No case of Infectious or Contagious Disease.
(b) No illness exhibiting Feverish Symptoms or Diarrhœa, on board this Vessel
while lying in the Port of
or during the voyage here (except as stated on the back hereof).
I have inspected every person on board within 12 hours of arrival, and find all in
good health and free from symptoms of Cholera, Yellow Fever and Plague.
