202 (3) and (4).);
to make a return to the Town Clerk for transmission to the Board of
Control within seven days after the end of each quarter of all poor persons
of unsound mind visited by him during the preceding quarter, or, if he has
no knowledge of any such cases, to make a return to that effect. (The
forms on which these returns are to be made will be supplied, but no fee is
payable).
Change of Medical Attendant.
A patient who once chooses his or her medical attendant shall be allowed
to change that attendant at the expiration of any calendar quarter, i.e.,
31st March, 30th June, 30th September, 31st December.
The Medical Practitioner shall be allowed to relinquish a patient at the
end of any calendar quarter if some other Medical Practitioner on the panel
will accept the case, otherwise the duties set out in this scheme shall in respect
of such patient be carried out by the Medical Practitioner initially responsible,
for a period of 12 months from the date of issue of the Medical Card.
Capitation Fee.
A capitation fee of 16s. per annum will be paid to the medical attendant
by instalments at the end of each calendar quarter, the first instalment being
payable at the end of the quarter in which the patient first receives treatment.
In any case where a patient changes his Medical Attendant or a Medical
Attendant relinquishes the patient, at the end of a calendar quarter, no further
instalment of the capitation fee shall be payable to the Medical Attendant from
whom the patient has transferred or by whom he is being relinquished.
In any case of a change of a Medical Attendant, payment of the annual
capitation fee to the new Medical Attendant shall commence as from the
quarter during which the patient first receives treatment, subject to the issue of
a medical card by the Relieving Officer.
Services of Nurse.
Any medical practitioner requiring the Public Assistance District Nurse
to call and attend to any patient, shall complete a requisition form for this
purpose, the form to state:—Name, address, age, illness, assistance required
(see Form (d) ). This form shall be sent to the Medical Officer of Health
at whose office the nurse will visit at such times to be arranged to collect
the forms and carry out the instructions contained therein. The nurse will
submit to the Medical Officer of Health a monthly return showing the daily
number of visits paid.
Extra Medical Relief.
Recommendations for additional nourishment must be forwarded to the
Relieving Officers (see Form (e) ), who will bring them to the notice of the
next Relief Sub-Committee—the Relieving Officer in the meantime granting
such "medical extras" on their own responsibility in cases of urgent necessity.
Admissions to Special Institutions or Convalescent Homes.
Where a medical practitioner recommends that a patient be sent either
to a Convalescent Home or for treatment in a special Institution, he shall
forward such recommendations to the Medical Officer of Health, who will
examine such patient and forward a report to the Public Assistance Officer
for submission to the Public Assistance Committee.
