All who live or work in Shoreditch are eligible to receive treatment at
this hospital. Persons who have resided in the Borough for six months (but
not non-residents working in the Borough) who state that they are unable
to afford the foregoing charges are asked to complete a form stating the
number of the family and the income. Applicants are further required to
sign the following undertaking:—
1, the undersigned, hereby acknowledge that in the event of my
application for treatment at the Municipal Dental Hospital without
present payment in full being granted, the Shoreditch Borough Council
reserve to themselves the right to call upon me for payment of the cost
of such treatment if at any time it is found that I am in a position to pay
for the same.

After verification by the Enquiry Officer this statement is used to fix the proportion of the full charge to be paid, the remissions being 25, 50, 75 and 100 per cent., in accordance with the following income limits approved by the Council.

The six months residential qualification to which reference has been
made does not apply to Maternity and Child Welfare cases who are entitled
to the appropriate remissions by the fact that they are referred to the Hospital
from one or other of the Welfare Centres.
There were considerable changes in the permanent staff during the year.
Mr. H. L. Davies, the Dental Surgeon, resigned his appointment in April,
and later in the year the positions held by the junior Nurse and two of the
Dental Mechanics became vacant.
The temporary arrangements made for carrying on the work of the hospital
were still in being at the end of the year here under consideration. At the
time of writing, however, steps are being taken to appoint a whole-time Dental
Surgeon. Other matters relating to the staffing and administration of the
Hospital stand deferred until this appointment has been made.
