Statutory Notice.
(A. 1.)
METROPOLITAN BOROUGH OF SAINT PANCRAS.
Public Health Department.
The Public Health (London) Act, 1891.—Dangerous Infectious Diseases.
Certificate of Medical Officer of Health.
This is to certify that the cleansing and disinfecting of
at , and any
articles therein, or the destruction of such articles, would tend to prevent or
check dangerous infectious disease.
Medical Officer of Health.
To the person in charge of the Patient, and
To the Master of the House,
Whereas, by a Certificate made by the Medical Officer of Health of the
Metropolitan Borough of St. Pancras (the Council of the said Borough being
the Sanitary Authority in and for the said Borough of St. Pancras, in the
County of London), (or a legally qualified Medical Practitioner) the said
Medical Officer of Health has certified that the cleansing and disinfecting of a
certain house (or of a certain part of a house), namely, "
of which said house (or part of a house) you are the
master (or being unoccupied you are the owner), situate at
,within the said Borough, and that the cleansing
and disinfecting of the articles therein likely to retain infection would tend to
prevent or check a dangerous infectious disease, to wit
Now, therefore, notice is hereby given to you as such master (or owner) as
aforesaid that the said house (or part of the said housed will be cleansed and
disinfected, and that the said articles will be cleansed and disinfected, or
destroyed, by the said sanitary authority, unless you inform the said sanitary
authority within twenty-four hours from the receipt of this notice that you will
cleanse and disinfect the said house (or part of the said house), and will cleanse
and disinfect or destroy the said articles to the satisfaction of the said Medical
Officer of Health, or of a legally qualified Medical Practitioner, within a period
of three days from the receipt of this notice.
And notice is hereby further given to you as such master or owner, that if
either (a) within twenty-four hours from the receipt of this notice you do not
inform the sanitary authority as aforesaid, or {b) having so informed the sanitary
authority you fail to have the said house (or part of the said house) cleansed and
disinfected, and the said articles cleansed and disinfected or destroyed within the
period of three days aforesaid, or (c) if you, without such notice, give your
consent, the said house or part will be cleansed and disinfected, and the said
articles will be cleansed and disinfected or destroyed by the officers, and at the
cost of the sanitary authority, under the supervision of the Medical Officer of
Health.
