which it is situated, or if not within the district, state where it is situated, and in what
district. Mark (w) the locality in which a workhouse is situated.
+ These age columns for notifications should be filled up in all cases where the Medical Officer
of Health, by inquiry or otherwise, has obtained the necessary information.
